Please complete the registration form below. Once your registration form is submitted you will receive an official Stage Players Welcome Letter via email. This welcome letter includes details about your child's acting class and a secured link to pay for classes.

 Liability Waiver and Assumption of Risk

 I, the undersigned, as the parent or legal guardian of the above-named minor (the "Participant"), hereby give my full consent for my child to participate in the acting class or program offered by Stage Players. I acknowledge that participation in an acting class involves physical movement and interaction with others, and while rare, there is a possibility of injury. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, which my child might incur as a result of participating in this program.

Medical Release

 In the event of an emergency, I authorize the Organization and its representatives to obtain medical treatment for my child, including first aid and emergency care, if necessary. I understand that every effort will be made to contact me before such action is taken.

 Photo and Media Release

I have read and fully understand this waiver and release and agree to be bound by its terms.
